That sounds like an issue for @Ohmer
No (full) log.txt file? (No log = no support, sorry).
Was installed/updated automatically (from VCV Library / VCV Rack 2 application), or manually by download from GitHub / manual copy .vcvplugin package file? IDK.
Strange because OhmerPrems v2.6.12 was beta-tested without issue by many users (all platforms), except global issue after OSX update to lastest Tahoe v26.5 (not always, but in some cases, for one other user). Why? IDK. Thanks Apple!
Uninstall/clean, then reinstall VCV Rack 2 from scratch solves the issue. Security? other defective plugin? Be sure Mac platforms are always a pain for most developers (never issues on Windows and Linux platforms, only Mac causes problems).
The truth is elsewhere.

Yeah, who knows, could be anything. @alblum : You should probably close Rack, delete the /Users/alblum_studio/Library/Application Support/Rack2/plugins-mac-arm64/OhmerPrems-2.6.12-mac-arm64.vcvplugin file, then open Rack again and let it update again to see if it helps. If not, and you’re on the latest macOS, maybe try and re-install Rack, like Ohmer indicates.
